Dermatology Associates accepts a wide variety of insurance providers, plans, and coverage for your skin health care. Please make sure you understand the details of your insurance coverage and contact your insurance provider directly if you are unsure about what part of your visit may or may not be covered.
Referrals: Many insurance plans require official requests from a patient’s Primary Care Provider (PCP) to your insurance company that your visit to our office be covered. These referrals must be in place prior to your visit or you may be responsible for covering your service. Please check with your insurance provider and submit your referral request to your PCP before scheduling your appointment. If your referral is not in place at the time of your visit you will have to sign a waiver to receive service.

Your insurance information will be verified at each appointment. Please make sure to bring your most current insurance card, copay, and credit card with you. Please also make sure your referral, if applicable, is in place prior to your visit.

*These plans are offered through the MA Health Connector. Dermatology Associates, P.C. does not accept Commonwealth Care but can take Commonwealth Choice if we are contracted with the company sponsoring the plan.
At each appointment, we will verify your insurance information. Please be sure to bring any insurance cards with you to our office.
